How do I make, change or cancel an appointment?
Please call our office during office hours for appointments. After office hours please leave a detailed message on our voice mail and we will get back to you with an appointment time.
Are there any charges for canceling an appointment?
Please call us to cancel atleast 24 hours before your appointment time. If you do not cancel, there will be a minimum fee.
What insurance plans do you accept?
We accept most insurance plans - PPOs, HMOs, Medicare and Medical.Do I have to pay for my visit at the time of my appointment?
You have to make a co-pay for during each visit depending on your insurance plan.

Who should I contact if I get sick after office hours or on weekends?
Call the main office number. You will be connected to our answering service. There is a nurse or physician on call after hours and weekends.
